- জাতীয় পরিচয়পত্রের কপি
- Bangladesh Medical and Dental Council (BMDC) রেজিস্ট্রেশন
- MBBS সনদপত্র
- SSC পাশের সনদ
ভর্তির সময় সব কাগজপত্রের স্ক্যান কপি ও হার্ড কপি অফিসে জমা দিতে হবে।
সার্টিফিকেট
কোর্স শেষে প্রতিষ্ঠানের সনদপত্র প্রদান করা হয়।
